INSURANCE
All full time regular employees are
entitled to major medical and dental insurance after a 30 days
waiting period. Details for our major medical and dental policy are
communicated to employees in the group insurance booklet. Master
Pumps offers major medical, dental, long term disability, and life
insurance. Continuation of medical and dental benefits through COBRA
is offered for those employees that find themselves unemployed,
spouses divorced and disabled. For more information on our benefits
program contact your supervisor or the Human Resource
Department.
JURY DUTY
All full time regular employees called
for jury duty will receive their regular rate of pay, minus jury
pay, for hours scheduled, but in no event will the make up pay
exceed 40 hours a week. An employee serving on jury duty will
present the official court check and other documentation to their
supervisor to be forwarded to the Payroll Department.
LEAVES OF
ABSENCE
There are circumstances that make it
necessary for employees to request time off from work activities.
This policy describes the specific circumstances and the criteria
that management will use to evaluate all leave of absence
requests. Medical leaves of absence require a Doctors Statement for taking Disability and also to be released to return to work for the same.
All full time regular employees are
eligible to request a leave of absence, which is defined as leave
tank other than as normal paid sick leave or scheduled vacation
leave.
Requests for leave of absence will be
evaluated and granted on approval by management for a maximum of six
weeks. If, at the end of the six week period, there is need for
additional time, and extension may be considered on submission of
proper documentation, e.g., a doctor's certificate. Request for
leaves and for extensions must be made in writing at least two weeks
before the starting date. |
