PERSONAL LEAVE
POLICY
Personal leave is accumulated at 1/2
day bi-weekly to all full time employees. Upon completion of 90 days
employment you may take the leave you have accumulated. Two
consecutive weeks leave may be taken with special authorization from
your supervisor. Two weeks notice to your supervisor is required
prior to taking more than three days of personal leave. The last
five days of leave accumulated from the preceding year may be
carried over to the next year in the event that all of your leave
was unable to be taken in the year in which it was accumulated. The
carryover days are charged off on a first earned, first out basis as
personal days are paid and used. Your particular department may have
difficulty in taking leave at certain times during the year.
Therefore, plan ahead so that personnel will be
adequate.
Personal Leave benefits may only be
taken while employed with the Company. If your employment with the
Company is terminated for any reason, you will not be paid any
personal leave benefits that have not already been taken prior to
your termination.
Personal Leave benefits are not earned
and therefore not paid on any final paychecks who have been
terminated or who have formally resigned or taken unauthorized
leaves of absence. Any employee not showing up for work during his
last pay period will not be counted for those days of activity; nor
benefits paid for personal leave.
Employees not returning for work after
scheduled personal leave or any other absence will not be paid for
personal leave on his or her last pay period.
The employees' last day will be
considered to be the last full day of work the employee performed
with termination occurring that same date. The Company does not have
a Vacation Policy and this Personal Leave Policy is not to be
construed as a Vacation Policy of any sorts.
WORKERS'
COMPENSATION
Regardless of the nature of severity,
all injuries sustained on the job must be reported to your
supervisor at once. Any employee who fails to report an injury
during their shift will be subject to disciplinary
actions.
The Company insures employees against
accidental injuries under the State Worker's Compensation Act. Each
State has a specific form that needs to be filled out and turned
into the Human Resources Department within days after the injury.
Direct all inquiries and bills to the Human Resource
Department.
Your major medical and dental insurance
will continue as before with the employee paying half the monthly
premium payments up to three months. After three months the employee
will have the option of continuing their insurance on
COBRA.
