STATUS
Employers are to report promptly,
changes in any of the following to their Branch
Managers:
1. Name 2. Address 3. Phone
Number 4. Marital Status 5. Dependents 6. Formal
Education 7. Courses Completed/Other Training or Skills
Acquired 8. Selective Service Status 9. Person(s) to Notify in
Case of Emergency 10. Physical or other limitations that may
effect their ability to perform the essential functions of the
job 11. Beneficiary
Branch Managers are responsible for
forwarding such information to the Human Resource
Department.
